READ THE FOLLOWING CAREFULLY:
What we need you to bring to your child's appointment:
- New Patient please download and fill out Forms (PDF) (Required for new patients)
- New Patient – custodial parent must bring the child to the first appointment. If you are not the custodial parent you must bring court paper
- Insurance Card
- Parent/Guardian drivers’ license or state Id
- Patient Social Security number
- Referral
- Current Injury – Recent X-Rays (film or CD)
- Prior Injury or condition – All previous X-Rays (film or CD) and previous treatment records.
- If you don’t speak English, you must bring an adult translator (over 18 years old).
Lo qué necesitamos qué traiga a la cita de su niño:
- Tarjeta del seguro
- Paciente Nuevo – El padre que tiene custodia tiene que traer el menor a su primer appt..
- Si no eres el padre del menor tienes que traer papeles del la corte o de custodia legal.
- La licensia ó identificación del padre/guarda
- Número de la Seguridad Social paciente
- Remisión Lesión actual – radiografías recientes (película ó CD)
- Lesión o condición anterior – todas las radiografías anteriores (película ó CD) y expedientes anteriores del tratamiento.
- Si usted no habla inglés, usted debe traer un traductor adulto (mayor de 18 años).
